EPISODE271 - Janet Stout, PhD -Legionella Issues & Answers
Dr. Janet E. Stout is director of Special Pathogens Laboratory, and a research associate professor at the University of Pittsburgh Swanson School of Engineering in the Department of Civil and Environmental Engineering. A clinical and environmental microbiologist, Dr. Stout is internationally recognized for more than 30 years of pioneering research in Legionella. Credited as the first to demonstrate the link between Legionnaires' disease and the presence of Legionella in hospital water systems, her seminal discovery in 1982, shifted the focus from cooling towers to water distributions systems as the primary source for Legionnaires' disease. This changed the direction of research and expanded the water treatment industry's approach to controlling Legionella. Since then, Dr. Stout has evaluated every major Legionella disinfection technology used today. An advocate for prevention, Dr. Stout was instrumental in developing the first U.S. prevention strategy for hospital-acquired Legionnaires' disease in 1993. Later adopted by the Veteran's Health Administration and others, the guideline continues to serve as a national model for Legionella prevention. In addition to Dr. Stout's numerous research articles published in major medical and scientific peer-reviewed publications, she co-authors the Legionella chapter in Hospital Epidemiology and Infection Control , Harrison's Principles of Internal Medicine, and the Manual of Clinical Microbiology. Active in scientific and professional associations, Dr. Stout serves on the Legionella standard committee for the Cooling Technology Institute and on the American Society of Heating, Refrigerating and Air Conditioning Engineers' committees for Legionella Guideline 12 and the proposed ASHRAE Legionella standard.

Pertti (Bert) Hakkinen, Ph.D., is the Acting Head of the Office of Clinical Toxicology, and the Senior Toxicologist and Toxicology and Environmental Health Science Advisor in the Division of Specialized Information Services, National Library of Medicine (NLM), National Institutes of Health (NIH). Dr. Hakkinen is the former project leader for the Wireless Information System for Emergency Responders (WISER) and the current project leader for the Chemical Hazards Emergency Medical Management (CHEMM) tools, represents NLM on various committees, and provides leadership for NLM's participation in national and international efforts in toxicology-, exposure-, and risk assessment-related information. Dr. Hakkinen received the 2011 "Risk Communication Excellence Award" from the Alliance for Chemical Safety for his work on the CHEMM project, and received one of the annual "NIH Director's Awards" (August, 2011), with this award recognizing a role in efforts led by NIEHS related to the Deepwater Horizon Gulf Oil Spill. Dr. Hakkinen has been an invited expert or reviewer for several U.S. EPA-, Health Canada-, and European Commission-sponsored efforts to develop or revise human exposure assessment guidance and resource documents and software, and was selected as a member of the core panel of experts in 2005-2007 for the Voluntary Children's Chemical Evaluation Program (VCCEP, a part of the Chemical Right-to-Know Initiative of the U.S. EPA). He is a co-editor and co-author of the latest edition of the Encyclopedia of Toxicology, and of the last two editions of the Information Resources in Toxicology book. Stephanie Publicker has an Associate Degree in Nursing, a BA in Italian and a Masters in Library Science. Since receiving her MLS from the University of Pittsburgh in 1991, she has been involved in end user training, getting her start when users were thrilled to have a 2400 Baud modem. She currently works in the National Library of Medicine's Specialized Information Services Division where she is involved in end user training, database development, and web content management. Dr. Catherine Noakes, PhD, CEng is the Director of Pathogen Control Engineering Institute at the School of Civil Engineering, University of Leeds. She is also the Leader of Aerobiology and Infection Control Research Group at the University of Leeds. The Aerobiology and Infection Control Group is a multi-disciplinary research team comprising microbiologists, engineers and mathematical modelers. The group has considerable experience in evaluating bioaerosols in indoor environments and engineering measures to control infection. Their Research is driven by the continuing problem of hospital acquired infection and the worldwide health concerns about infectious diseases spread through airborne routes. Airborne transmission is known to be a primary mechanism in the transmission of TB and influenza, but has also been implicated in the transmission of nosocomial infections including MRSA, Acinetobacter spp, C. difficile and norovirus. In addition, contamination of the environment through contact with dirty hands and objects or airborne dispersal provides a potential reservoir of pathogens which could subsequently cause infection by indirect contacts. Regardless of the pathogen, successful control of infection involves breaking the chain of transmission, for which it is necessary to understand both the mode of transmission as well as the nature of the pathogen and its behavior in the environment. Understanding this complex interaction between people, pathogens and the built environment is at the heart of their research. The paper that prompted the press interest is work carried out by Marco-Felipe King, one of my PhD students, stated Dr. Noakes. His overall project is trying to determine whether single rooms in hospitals present a lower infection risk than multi-bed rooms. This is something that is quite often stated and for which there is anecdotal evidence, but isn't quantified. He is trying to quantify the difference in risk between the two types of rooms for infection spread due to environmental contamination. This paper presents the first part of this work where he was looking to quantify surface contamination due to an aerosol source and validate the techniques for modeling it. This is feeding into the next stage of the research where he is using the ability to predict contamination on a surface together with a mathematical model for a nurse moving around a ward and touching surfaces to compare the potential for contamination.

Larry Carlson joined Therma-Stor in 1992 as product manager for ventilation and dehumidification products. In 1994 Larry introduced the Phoenix 200, the first LGR dehumidifier. Through 2003 he oversaw the expansion the Phoenix Restoration Equipment product line to include the Guardian HEPA Systems, Focus axial air mover and Phoenix 300 and 200 MAX. After 2 years at Dry Air Technology where Larry assisted in the creation of the Atlantic LGR and Force9, he returned to Therma-Stor in 2006 as marketing manager. Currently Larry is the Phoenix Restoration Industry Manager and International Sales Manager. Larry has been a member of RIA since 1994 and the IICRC since 1998. He participated in the creation of the Applied Structural Drying Technician (ASD) certification and is currently serving on the IICRC S-500 Consensus Committee which is creating the 4th edition of the S-500 Standard and Reference Guide for Professional Water Damage Restoration.

Mr. Kahane received a B.A. in Physiology and an M.P.H. in Biological and Environmental Health Sciences from U.C. Berkeley. He has been a practicing Certified Industrial Hygienist for the past 18 years. Mr. Kahane provides technical expertise and litigation support for asbestos, heavy metals, fungal and other IH-related exposure cases. He performs and supports industrial hygiene and indoor air quality investigations for a variety of commercial, industrial, insurance, military school and residential clients. He has performed and is well versed in various analytical methods including optical microscopy, XRF and AAS/ICP. Mr. Kahane is often involved with the statistical evaluation of survey data, and the experimental design for a variety of indoor exposure assessments. Industrial Hygiene Consulting, Inc. (previously operating as Jacobs Environmental, Inc.) was founded in 1997 by Mr. Bruce Jacobs, who continues on today as President and Principal Consultant. Mr. Jacobs has accumulated 38 years of experience in the environmental and safety & health profession. Mr. Jacobs developed gas chromatographic analytical methods, and validated some of the initial uses of diffusion-based sampling and analytical methods for air contaminants, as a Chemist for the U.S. Army. Up to 2006 Dr. Ammann was the Senior Toxicologist: Air Quality Program, Washington State Department Ecology and prior to that she served as the Senior Toxicologist: Office of Environmental Health Assessment Services, Washington State Department of Health. Before serving with the State of Washington, Dr. Ammann participated in the U.S. EPA's Hazardous Air Pollutants team that was developing guidance for non-cancer risk assessment. Dr. Ammann served as vice-chair of the Bioaerosols Committee of the American Conference of Governmental Industrial Hygienists (ACGIH) from 1997-2003. She authored the chapter on Microbial VOCs in the ACGIH book, Bioaerosols, Assessment and Control, and contributed to several other book chapters. She also served as assistant editor to the book. Dr. Delia received her B.S. in Chemistry from Kalamazoo College, and her M.A. and Ph.D. degrees in Atmospheric and Oceanic Sciences from the University of Colorado. In addition, she is proficient in the operation of Gas Chromatograph-Mass Spectrometer (GC-MS) instruments and performing the chemical profile interpretation of the GS-MS data. During her six-year tenure at Prism, Dr. Delia has been responsible for the development and deployment of several new methodologies for air quality testing in homes. Jack D. Thrasher was educated in California with a B.S. in zoology/chemistry and a Ph.D. in human anatomy/cell biology from UCLA. He taught human anatomy, histology, physiology, and embryology at the University of Colorado, School of Medicine at Denver and at the UCLA School of Medicine in the 60s and early 70s. He was consulting toxicologist that did testing on the safety and efficacy of medical devices, drugs and cosmetics from the mid 70s through the mid 80s. Since 1984, he has consulted in Environmental Toxicology, Immunotoxicology and Human Health effects from toxic exposures to private citizens as well as practicing physicians. He has collaborated in many scientific peer reviewed papers on formaldehyde, mold and mycotoxins, hydrogen sulfide, and insecticides. He is an author or co-author on the adverse health effects (neurological and immunological) in humans following exposure to damp indoor spaces. His most recent manuscript published in Toxicology and Industrial Health is titled "The biocontaminants and complexity of damp indoor spaces: more than meets the eyes". This manuscript reviews molds, gram negative and positive bacteria, mycotoxins, endotoxins, 1-3-beta glucans, galactomannans, particulates and MVOCs present in damp indoor spaces. All of these biocontaminants impinge upon occupants affecting various organ systems and more likely than not have synergistic toxic actions. Dr. Thrashers areas of expertise include: Formaldehyde in trailers and Mobile Homes; organophosphate pesticides, chlorinated pesticides; herbicides, pyrethroids, organic solvents including chlorinated (TCE, PCE); Indoor air microbial (fungi and bacteria) and their by-products in water damaged homes, public buildings and schools; and hydrogen sulfide. He has testified as an expert witness in many legal cases for plaintiffs and defendants.

Ultra Violet Devices Inc. (UVDI) is one of the oldest and most respected ultraviolet disinfection companies in the world. It all began with the hard work and ingenuity of a single man. Three generations ago Luis Veloz was an engineer for Westinghouse when their R&D engineers developed and patented the first commercial ultraviolet lamps. Luis was excited by this new technology and in 1949 he purchased a Westinghouse UV lamp dealership. Early applications were difficult to develop, but Luis had a vision of how ultraviolet light might be widespread and effective. He formed Aquafine Corporation to focus on water disinfection. In 2003, UVDI launched its ALTRU-V line of air disinfection products. We have talked a good bit with past guests about the potential benefits and drawbacks for specific airstream UV disinfection with IAQ experts for three years it is time we hear from a respected manufacturer for their perspective. Dr. Horner is the laboratory director and a principle consultant for Air Quality Sciences. He has a Ph.D. in plant pathology from Virginia Polytechnic Institute, an M.S. in mycology from the SUNY College of Environmental Science and Forestry in Syracuse, and a B.A. in botany/comparative literature from the University of North Carolina. After receiving his Ph.D., Dr. Horner completed an NIH funded fellowship characterizing fungal allergens at Tulane University School of Medicine before joining the research faculty in the Allergy/Clinical Immunology Section, where he established a bioaerosols and indoor mold research program. He served as the chair (2002) of the Environmental Microbiology Laboratory Accreditation Committee of the American Industrial Hygiene Association Lab Quality Assurance Program, and served on the Analytical Accreditation Board through 2003. He is a member of ASTM International sub-committee D22.08 on sampling and analysis of mold and sub-committee G03.04 on biological deterioration. Dr. Horner has conducted investigations and complaint resolutions in courthouses, commercial buildings, hospitals, residences, schools and defense manufacturing facilities. He is also a Fellow of the American Academy of Asthma Allergy and Immunology and a LEED AP.
